How to Manage Patient Consent Forms

Digital consent forms streamline the consent process, reduce paper, and maintain a clear record of patient authorization for treatments and data use.

MyDentalPractice supports several consent form types:

Form TypePurpose
Treatment ConsentAuthorization for a specific dental procedure
General ConsentBroad consent for routine care
Medical History ConsentPermission to collect and store health data
Photography ConsentPermission to take clinical photos
Data Processing ConsentNDPR compliance for personal data handling
  • Go to Settings → Templates → Consent Forms
  • Click + Create Template
  • Choose a template type or start blank
  • Use the editor to add:
  • - Title and description

    - Consent text with legal language

    - Checkboxes for specific items (e.g., "I consent to X-ray imaging")

    - Signature field

    - Date field

  • Click Save Template
  • In the Office

  • Open the patient record
  • Click Consent Forms tab
  • Click + Request Consent
  • Select the form template
  • Hand the device to the patient or display on a tablet
  • Patient reads the form, checks required boxes, and signs digitally
  • The signed form is stored in the patient's Documents tab
  • Via Patient Portal

  • From the consent forms list, click Send via Portal
  • The patient receives a notification to review and sign the form online
  • Signed forms appear in the patient record automatically
  • Before Treatment

    Configure mandatory consent at Settings → Treatments:

  • Edit a treatment type
  • Toggle Require Consent Before Treatment
  • Select which consent form template to use
  • The system will prompt for consent before marking the appointment as "In Progress"
  • Managing Signed Forms

    View all consent forms for a patient in their Consent Forms tab:

  • Signed forms are marked with a green checkmark and timestamp
  • Pending forms show as orange
  • Expired forms (if set to expire) show as red
  • Click any form to view the full signed document
  • Set consent forms to expire after a period:

  • Annual renewal for general consent
  • Per-procedure for treatment consent
  • The system alerts you when a consent is about to expire
  • Exporting and Printing

  • Click Print on any signed form to generate a PDF
  • Click Export All to download all consent forms for a patient as a ZIP file
